METHODS OF PAYMENT

Payment may be made by credit card only. Payment is expected at the time of your session.

DOCUMENTATION AND REPORTS

Requests for the creation of reports, assessments, treatment summaries, letters, or other materials that extend beyond the request of ordinary medical records will have an additional cost that is outlined in our Fee Agreement form that is required to be completed before services are rendered. Fees for documentation may be waived at the discretion of the counselor and the practice. Insurance plans do not provide coverage for these fees. We are not able to honor every request for documentation or reports.  

INSURANCE

Resilience Center is currently in-network with BlueCross Blue Shield, Healthchoice, Healthcare Highways, Cigna, United Healthcare (select plans) and Aetna (select plans). If you have out-of-network benefits, we are also able to provide the documentation that your insurance company requires for reimbursement of out-of-network services rendered. 

CANCELLATION POLICY

It is understandable that appointments need to be canceled and rescheduled on occasion. It is required that you give at least 24-hours advance notice when canceling an appointment. This is because the practice needs time to be able to make the appointment time available to other individuals. The practice reserves the right to charge $35.00 for sessions that are canceled without 24 hours of notice.
